Patient Consent or Contact by Email Form The DoH recommend emailing patients and asking them to complete your online survey is one of the best ways to get a strong response.

Our secure online form captures the patient's consent, age group, ethnic group, email address etc. It mirrors the paper version of the form recommended by the DoH.

Data is stored ready for sending bulk emails to all patients [see below].

For your own analysis the data can also be downloaded and imported into products like Microsoft's Excel and Access.
