Comments & Suggestions
The comments form is designed to be an online version of the 'Patient Comments / Suggestions' box in reception. It enables patients to send in any ideas or suggestions they have about the practice. Comments be sent anonymously or name / email etc can be supplied.
Anti-Spam:
The comments form includes an anti-spam application to help stop unwanted comments being sent to the practice.
