Introduction:
This allows patients to inform you of an address or telephone
number change using an online form. These are sent to you securely and the patient has the option of sending a telephone
number change only, or an entire address change. Both the old and new address must be entered.

Our web based control panel is used to report on any change of address / tel nos received.
